Documents You Need To Provide Us

For bankruptcy consultation:
  • Tax Returns or proof of income for past 3 tax years
  • Pay stubs for the past six months or other proof of income for the same period
  • If self employed: year-to-date profit and loss statement
  • Proof of other income such as child support and rental income
  • Recent copy of a statement from each creditor
  • Statements from each deposit account for the past 2 months
  • Recent statement from each retirement account
  • All outstanding loan documents including, home mortgages, automobiles, RV, & boats.
  • Property tax statement
  • Copies of any collection notice, default notice from lenders or collection agencies.

Prior to filing bankruptcy:

  • Each debtor must complete a course in credit counseling and provide us with a Certificate of Completion of Credit Counseling (credit counseling can be completed online with an approved provider)
  • Provide us with a credit report
  • You will need to continue to provide us with all pay stubs and evidence of income up to the date of filing the bankruptcy petition.

After filing bankruptcy:

Each debtor must complete a course in financial management from an approved credit counseling agency and provide the court with a signed Statement of Completion  prior to receiving a discharge of debts.
